This website was developed and is maintained by the Child and Adolescent Health Measurement Initiative (CAHMI).
This website is funded by The Commonwealth Fund (www.cmwf.org) and is available for free thanks to their generous support.
The website has two purposes:
1) It is the place where parents can fill out the Promoting Healthy Development Survey (PHDS). Upon completing the survey, you will get a personalized report that is created based on your responses. This report will guide you in partnering with your child’s health care provider to improve your child’s health care.
2) It is also serves health care providers and health systems: Your responses to the Promoting Healthy Development Survey are kept confidential and shared with your health care provider in the form of a summarized report with other parents' survey results to help your provider understand how he/she can improve the care he/she provides to young children.
